The ConnectED program provides case coordination and linkages to clients with multiple and complex needs who present repeatedly to the Alfred Hospital Emergency Department and are in need of support. All referrals come through the Alfred Hospital.
ConnectED is a partnership approach comprised of staff members from:
- Windana
- Prahran Mission
- RDNS Homeless Persons Program
- Port Phillip Community Group
- Bayside Community Health
- Sacred Heart Mission
- Inner South Community Health Service
ConnectED works closely with mental health services, private psychiatrists, housing agencies and support services.
